QUESTION:

if someone has a medical issue (severe throat and ear problem with heavy and thick mucus which gets stuck and needs to gargle with water to feel well enough, otherwise does not feel well). Can they gargle during fast?

Yes, as long as the water does not go down the throat.

They must be extra careful though.
